Please nominate a person, group or congregation, within a conference, who has made a significant difference through ministry, who has developed new ministries, or who has exemplified the Lutheran faith through their service and ministry.
The Award Committee will consider the following as they choose which nominations from each conference to award.

- Relationship between faith and service
Ministry takes on a variety of forms by proclaiming the Gospel in innumerable ways. Examples are:
- Drama
- Music
- Neighborhood Outreach
- Mission Outreach
- Innovative Worship
- Social Ministry
- Ecumenical Endeavors
The Award Committee will select dedicated nominees from each conference who collectively represent a wide variety of services and service deliveries.

Make checks for $20.00/person payable to Carroll Lutheran Village, denoting “Ministry Dinner.” A vegetarian option is available upon request. One Dinner RSVP form per person please.
If you have several members in your group, an individual RSVP should be filled out for each attendee. Please note if a group is nominated, only one individual from that group will receive a free meal. Seating is limited; requests for reservations must be accompanied by payment. Reservations after Monday, April 16, 2011 will be assessed an additional $5.00 per person.
